Always say I accept your apology instead of saying "it's ok" so that they don't think it's ok to do again. It's also ok to say "Thank You" as a response.
It is perfectly acceptable to acknowledge that someone has apologized, and still refuse to accept their apology. There are things in this world that cannot be apologized for.
